New Patient Appointment Policy
Welcome to Florida Healthcare System. Before scheduling your first appointment, please complete our New Patient Intake Form, including medical history, consent for treatment, and insurance details. You must provide a valid ID and insurance card (if applicable).
Cancellation & No-Show Policy
- Appointments must be canceled at least in advance.
- Late cancellations or missed appointments will incur a fee (unless due to an emergency).
- After missed appointments, future scheduling may be restricted and you will be discharged from the practice.
Rescheduling Policy
Patients may reschedule appointments via phone, text, or online portal. We require at least notice.
Tardiness Policy
If you arrive more than late, we may need to reschedule your appointment. This will be considered a missed appointment if the provider cannot accommodate you.
Walk-in Policy
We do not accept walk-ins. All visits must be scheduled in advance.
Payment Expectations
- All payments are due at the time of service.
- For telehealth visits, payments must be completed before the appointment begins.

Insurance & Billing Policy
- You can review our current list of insurance providers here.
- Patients are responsible for co-pays, deductibles, and non-covered services.
Out-of-Network & Self-Pay Policy
- Out-of-network patients must pay in full at the time of service.
- Self-pay rates are available upon request.
Late Payment & Collections Policy
Unpaid balances exceeding will be subject to collections. A late fee may apply.
Medication Refill Policy
- You must submit a refill request at least before your prescription ends.
- Refill requests require for processing.
- No refills will be provided outside of business hours.
- Patients must attend follow-up appointments as required for refills.
Controlled Substances Policy
- Controlled medications will only be prescribed under close supervision.
- Lost or stolen prescriptions will not be replaced early.
- Patients may be required to sign a Controlled Substance Agreement.
Prior Authorization Process
If your medication requires prior authorization, we will work with your insurance, but approval is not guaranteed.
HIPAA Compliance & Patient Privacy
Your medical records are protected under HIPAA. We do not share your information without written consent, except as required by law.
Release of Medical Records
Requests must be submitted via the Medical Records Release Form. Processing takes , and a fee may apply.
Communication Policy
- Secure messaging via Patient Portal is preferred.
- Email and text communication are limited to appointment reminders.
Scope of Practice
We provide psychiatric evaluations, therapy, and medication management. We do not provide forensic evaluations or disability determinations.
Termination of Treatment Policy
We reserve the right to discontinue treatment if a patient:
- Misses multiple appointments without notice.
- Does not adhere to prescribed treatment plans.
- Engages in abusive or inappropriate behavior toward staff.
Crisis & Emergency Care Policy
For immediate crises, call 911 or go to the nearest ER. We do not provide 24/7 crisis support.
Code of Conduct
All patients must treat staff and other patients with respect. Disruptive behavior, including threats or verbal abuse, will not be tolerated.
Violence & Harassment Policy
Threats, violence, or harassment will result in immediate termination of treatment and possible legal action.
Children in the Office Policy
Children under 12 should not accompany patients unless necessary.
Eligibility for Telehealth
Telehealth appointments are available for follow-up visits and medication management. However, initial evaluations and urgent situations may require in-person visits.
Technology & Location Requirements
- Video & Audio Required – All telehealth appointments must use both video and audio. Audio-only sessions are not permitted.
- No Moving Vehicles – Patients cannot attend appointments while driving or riding in a moving vehicle, even if they are a passenger.
- Private & Quiet Location – Patients must be in a private, quiet room without distractions. Public places (e.g., coffee shops, stores, workplaces) are not allowed.
- Stable Internet Connection – A strong internet connection is required to avoid disruptions during the session.
No-Show & Late Cancellation for Telehealth
- Patients must cancel or reschedule at least in advance to avoid a cancellation fee.
- If a patient logs in from an unapproved location (moving vehicle, public space, or noisy environment), the session may be canceled and subject to the no-show policy.
Informed Consent Policy
All patients must sign a Consent for Treatment Form before starting care.

We are required by law to report:
- Child or elder abuse
- Suicidal or homicidal threats

- Allows us to securely charge your card for copays, deductibles, and fees for no-shows or late cancellations.
